Level One, Self and I/O Waiver Provider

Our Supported Living Program offers individualized personal support that enables individuals to maintain their independence. Supported Living is offered to children and adults that are on I/O, Level One or Self waivers. These Services are customized to meet the individual needs of the individual. Staff assist individuals in completing their everyday tasks.
